Dynamic Forms Login Issues

Students:

Student: Dynamic Forms uses Single Sign-On, so you will need to be logged in to BC One Access for Dynamic Forms to Auto Log-in to the student into Dynamic Forms.

Log in to BC One Access and then go to the form by using the Broward College web page or to a link in the student’s BC email.

When you click on a Dynamic form and is not already logged into BC One access, BC One Access Log in screen will display. Use your BC Credentials to log in and then it will take you to the Dynamic form.

The user will click on the Complete This Form to access the form.

Note: If you see your Name: Let’s get started and your name and Complete This Form, you are in. Some do not realize they are in as their name does not display on the top right-hand corner.

If the user is looking for a status of a form the user can select Pending / Draft Forms or Forms History.

Parent Login:

Some forms require a Parent or legal consent, the student provides the Parent email address in a form in Dynamic forms. The parent receives an email to their Personal email account that the student provided. The student will need to log out of Dynamic forms and BC One Access or use a different web browser and then the Parent or legal guardian can click on the link and create an account in Dynamic forms. The parent only needs to create an account once and can use the same credentials to log in for other forms.

Forms are not listed in Dynamic Forms. Forms can only be accessed through a web link or a link in an email.

Appeal of Residency Denial - This form is for a student who wishes to appeal the denial of residency for tuition purposes. The student must seek classification/reclassification at the campus prior to submitting a request to the college-wide appeal committee.

Florida Classroom Teacher Tuition Fee Waiver - Fee waiver for teachers employed full time by a school district in the state of Florida.

Florida State Employee Tuition Fee Waiver - This form is for Florida state employees seeking a tuition fee waiver for courses taken at Broward College. Students seeking this waiver must complete a Broward College application for admission and complete all admission requirements. Registration begins for Florida state employees on the date listed on the registration dates calendar.

Home School Completion Affidavit - A student who is claiming that home school graduation requirements are complete.
